1. Who we are
OnVerra Health, LLC is a New Jersey Health Care Service Firm registered with the NJ Division of Consumer Affairs. We provide non-skilled home care in clients' homes in New Jersey. All hands-on care is given by Certified Homemaker-Home Health Aides (CHHAs) working under the supervision of a New Jersey-licensed registered nurse, our Health Care Practitioner Supervisor.
| Office | 2414 Morris Avenue, Suite 352, Union, NJ 07083 |

2. Services we provide
| Service | What it includes | Provided by |
|---|---|---|
| Personal care | Help with activities of daily living: bathing, grooming, oral care, dressing, toileting and incontinence care, transfers, walking and mobility, positioning, eating, and help with an exercise program the plan of care lists. Reminders to take medication and help with self-administration. | CHHA |
| Homemaker and environmental support | Light housekeeping in the client's living areas, laundry and linen changes, meal planning and preparation (including a therapeutic diet in the plan of care), shopping and errands. | CHHA |
| Companion care | Conversation and social activity, reading, supervision for safety and well-being, and going with the client on walks, errands and appointments. | CHHA |
| Respite care | Any of the services above, scheduled to give a family caregiver a break. | CHHA |
| RN evaluation and supervision (part of every service, not a separate skilled service) | An in-home RN evaluation before care starts; a written plan of care; instruction of the CHHA in the client's tasks; plan-of-care review at least every 30 days; an in-home RN evaluation at least every 60 days; and reassessment when the client's condition changes. | Registered nurse |
Every task a CHHA performs is listed on the client's plan of care and is within the scope of New Jersey CHHA certification.
3. Services we do not provide
- Skilled nursing care of any kind, including medication administration, injections, wound care, catheter or tube care, and any task that requires a licensed nurse to perform.
- Therapy (physical, occupational or speech), medical social work, hospice, or any service that needs a physician's order.
- Medication administration by aides. CHHAs give reminders and help with self-administration only; they do not administer, pre-pour or change doses.
- A CHHA training program. OnVerra does not operate, sponsor or partner in a homemaker-home health aide training program. We hire only aides who already hold current CHHA certification from the New Jersey Board of Nursing.
- Uncertified caregivers. We do not place anyone to provide hands-on care who is not a certified CHHA.
If a client needs a service we do not provide, we tell the client or their representative and help them find a provider who offers it.
4. Who we can serve
We accept a client when our registered nurse has evaluated the client in their home and there is a reasonable expectation that we can meet the client's needs safely with the services in Section 2 and the staff we have available. If a client's needs change so that we can no longer meet them, we tell the client in writing, with the reason and the date services will stop, and help arrange a transfer.
5. Payment
OnVerra is not currently enrolled with New Jersey Medicaid, any managed care organization or any insurance company, and does not bill them. The charges, if any, and how they are paid are set out in each client's written service agreement before care begins.
6. Questions and complaints
Anyone may ask about our services or make a complaint to OnVerra, to CHAP (Community Health Accreditation Partner), or to the New Jersey Division of Consumer Affairs, without fear of reprisal. Contact details are in the Client Bill of Rights given to every client at the start of care.
